Sesamoiditis: Small Bones Can Cause Big Problems

There are two small bones in your body called the sesamoids. They are located on the underside of your foot near the big toe and are about the size of corn kernels. They provide a smooth surface over which your tendons slide, assisting these tendons in transferring force from your lower leg muscles to your big toe and aiding your foot in bearing your body weight. When these bones become fractured or their attendant soft tissues become irritated, they are unable to fulfill their roles and you are left with a condition called sesamoiditis.

Sesamoiditis is most commonly seen in athletes including runners, baseball catchers, and ballet dancers.  Those with flat feet, highly flexible feet, or bunions are also at risk. Chronic overtaxing of the sesamoid bones and the tendons in the area are the most common cause of the ailment. Other known causes of sesamoiditis include hyperextension of the big toe, trauma to the foot, and long-term wearing of high-heeled shoes.

Signs and symptoms of sesamoiditis include:

pain on the underside of the foot, particularly at the ball of the foot

swelling

bruising

limited mobility at the big toe

limping, walking on the outside of your foot caused as you compensate for discomfort

referred pain in the knees, hips, or low back

If your doctor confirms your suspicion of sesamoiditis, he or she will have a wide range of treatment options available. Non-surgical approaches include medication for pain and swelling, steroid injections, new shoes, and orthotics (custom inserts to provide cushioning and support). Surgery is appropriate and necessary only in the most severe of cases.
