Getting Ready For Your Podiatry Appointment

PLEASE BRING TO YOUR APPOINTMENT

  • Insurance Cards

  • Photo ID

  • Credit Card - to be placed on file for any balances your insurance company deems your responsibility. We accept all major credit cards. We do not accept Care Credit.

  • List of Medications

  • Name and telephone number of your medical doctor

 
 

Arrival Time

  • If you have not checked in on line ahead of time you will need to arrive 30 minutes prior to your appointment time to complete the check in on a tablet provided by our office or on your mobile device. 

  • If you have checked in on line ahead of time we will need you to be in our office 15 minutes prior to your appointment in order to confirm your information and get you checked in timely. 

  • If you are late we will need to reschedule your appointment. There is a $75 missed appointment fee for failure to arrive timely or provide 24 hour notice of your inability to keep your appointment.
